How to See Us

The Ability Clinic is currently accepting new patients with a doctor referral. In certain cases, where an acute injury has occurred within 1 week, a referral is not required. Please call our clinic for more information.

 

To get a referral, please complete the following steps:

 

Paper method

  1. Download and print the referral form

  2. Bring it your Doctor to complete it (family doctor or specialist doctor)

  3. Scan the completed referral and email it to info@abilityclinic.ca or fax to XXX-XXX-XXXX.

 

Electronic method

  1. Have your doctor (family doctor or specialist doctor) access www.abilityclinic.ca

  2. Download and type into the referral form directly

  3. E-mail the completed referral form to info@abilityclinic.ca

What to Expect After Being Referred

  • Once the referral has been sent by your doctor, please allow us five business days to review it.

  • We will contact your doctor with an estimated wait time and ask for any additional details/reports, if required.

  • You will then be contacted via telephone/e-mail for scheduling. 

  • If you would like to also be placed on the cancellation list, please inform us.

You may schedule your appointment in one of two ways:

  1. Directly over the phone with our administrative staff, or

  2. Through the online booking portal, which requires a special code that remains active for 24 hours

 

If you have not heard from us, please:

  1. Ensure that the voicemail with your phone service provider is not full

  2. Ensure that e-mails are not being directed to your spam folder

  3. Call us directly if it has been longer than 5 business days

Questionnaires

  • Please complete the patient questionnaire e-mailed to you  

  • You may also complete a paper questionnaire in the clinic if you prefer

 

Imaging, lab work, and other tests

  • Please ask your doctor to forward any reports that may be relevant to the current issue

  • If you have physical copies of any reports or CDs, please bring this with you

 

Medications

  • Please ensure that you have an updated list of medications with you

  • Alternatively, you may also bring your medications with you

 

Personal

  • Please bring your Ontario Health Card for OHIP visits

  • Please avoid any scents as others in the clinic area may be sensitive to this

  • If you feel unwell, are experiencing fever, cough, or other symptoms, please contact us immediately for rescheduling

 

Accessibility

  • Our clinic was designed with accessibility in mind

  • Our halls are extra-wide to accommodate wheelchairs and other assistive devices

  • An ceiling Hoyer lift is available for patient transfers for those who may need them

  • A fully accessible, universal washroom is also available within the clinic

Languages

  • Currently, we are unable to provide any translation services

  • Patients are required to bring their own translator if unable to communicate in English

  • If you require a sign language interpreting services, either in-person or via video, please visit Canadian Hearing Services.

 

Parking

  • There is free parking on-site

  • Van accessible parking is available next to the main building doors

  • Please allow for enough time

  • Initial consultations can take up to 2 hours, depending on the complexity of not only your case, but cases ahead of you that may require additional time

 

All Appointments

 

  • Please bring a change of clothes, such as a tank top and shorts, for necessary physical examinations

  • If you are unable to do so, we will provide materials for you to change into

We accept cash, debit, and credit cards for payment of services that are not covered through patient insurance. We are unable to directly bill your insurer. Receipts will be given for insurance and tax purposes.


Acceptable third-party coverage includes the following: Workers’ Compensation Board, private extended healthcare insurance coverage, Canadian military, police forces and legal litigation.
